COOKEVILLE, Tenn. — Cookeville Primary Care Associates has received the highest possible recognition for care quality offered by the National Committee for Quality Assurance (NCQA). The Level III patient-centered medical home achievement demonstrates the commitment that Cookeville Primary Care makes each day in its approach to patient care through the patient-centered medical home (PCMH) partnership with BlueCross BlueShield of Tennessee.

“What we’ve seen across the board with our medical homes is the positive impact they have on care quality and improved patient outcomes,” said Jason Ross, director of physician network operations at BlueCross. “We’re seeing enhanced communication across the care team and improved coordination of the right care at the right time and in the right setting. Reaching PCMH Level III recognition is extremely difficult and we can’t say enough good things about the physicians and care teams at Cookeville Primary Care doing all the tough work to make this care model a real, sustainable success for our members.”

Cookeville Primary Care Associates joined the BlueCross PCMH model in the spring of 2009. It was one of the first practices to join an early pilot group. Currently BlueCross’ program is the largest in the state, with 29 participating practices, 60 sites and impacting more than 40,000 members.

In the BlueCross PCMH model, physicians partner with their patients for whole-person care. Treatment is better facilitated through technology, personal coordination and an active conversation between physicians, other members of the care team and most importantly, the patient. Improvements focused on proactive, preventative medicine, management of chronic conditions, adherence to medication, access to primary care rather than emergency care , and discussions about lifestyle changes and risk factors, are just a few of the things this model brings to the table.
