How to Apply

The State will select a limited number of qualified applicants to participate in the AccessTN program. Participants will be chosen by the State from those qualified applicants that return by mail a completed application.

Interested individuals must fit one of these specific situations:

Option 1 – Declined for health insurance by two different companies

Decline letters must be dated within the last 18 months, and included with the application.

Option 2 – Willing to go through medical underwriting process

In these instances, there is a $75 non-refundable application fee that must be submitted with the application and potentially, medical records could be requested at a later date.

Option 3 – Qualified pre-existing medical condition

If you have one of the 55 pre-existing medical conditions     approved by the State. In these cases, you must also complete a Physician’s Statement  form and return it with the application.

To apply, simply fill out an application  and return it by mail to the address on the right of this page. There is also an instruction booklet   to help you properly complete the application.
