Maternity Benefits

CoverTN plan members who become pregnant are eligible for maternity benefits under the CoverKids program.

To apply for these benefits, simply:

• Visit http://covertn.gov/web/coverkids_app.html 
• Print and fill out the CoverKids Application to apply for maternity benefits
• Print the Provider's Statement form, which must be filled out and signed by a physician to verify your pregnancy
• Mail both completed forms (CoverKids Application and Provider's Statement) to:
