CoverTN plan members who become pregnant are eligible for maternity benefits under the CoverKids program.

To apply for these benefits, simply:

• Visit www.covertn.gov 
• Print and fill out the CoverKids Application to apply for maternity benefits
• Print the Physician Statement form, which must be filled out and signed by a physician indicating your pregnancy
• Mail both completed forms (CoverKids Application and Physician Statement) to:
