Wellness benefits help keep you and your family healthy.

• Well-child care, to age 6, including appropriate immunizations, vision and hearing screenings, and other appropriate diagnostics.
• Annual well-woman exams including pap smears and any follow-up care.
• Mammograms.
• Prostate screening.
• Other preventive health services (age 6 and over - subject to $300 annual limit, per covered member).
  
  These services may include:

• Adult well care exam;
• Childhood immunizations;
• Blood pressure screening;
• Periodic cholesterol screening;
• Periodic sigmoidoscopy;
• Flu shot;
• Tetanus-diphtheria (Td) booster;
• Pneumoccocal immunization;
• Recommended adult immunizations and immunizations not received in childhood;
• Prescribed x-rays and lab screenings associated with preventive care;
• Speech and hearing screening performed by the physician during the preventive health exam; and
• Immunizations needed for foreign travel.

Exclusions are those wellness services not recommended by the guidelines of the
U.S. Task Force on Preventive Care and the medical policy of BlueCross BlueShield
of Tennessee.
