Personal Vision Plan Exam + Materials: $14.50 for each member enrolled

This plan offers coverage for an annual routine vision exam and materials for each member covered.

• Get an allowance of $100 for frames or contact lenses*
• Enjoy additional discounts on lens options and non-covered items*

Enrollment Information

• Personal VisionBlue is available with the purchase of a BlueCross BlueShield of Tennessee medical or dental policy or may be added to an existing BlueCross policy.
• Personal VisionBlue premiums are included on your monthly billing statement for each member covered under your existing BlueCross plan.
• If added, coverage includes all members on the policy.
• If terminated, Personal VisionBlue coverage is cancelled for all members on the policy and may not be re-added to the same policy.
• Personal VisionBlue is only available in Tennessee.
• Personal VisionBlue is not available to any member who does not have other BlueCross BlueShield of Tennessee coverage.
