Need to restore your health benefit plan?
If you don’t pay your monthly bill, your coverage may end. For Cover Tennessee, COBRA or individual members, you may qualify to have it restored.
COBRA coverage may be restored if this is the first time it ended for non-payment. You must make the request within 60 days of your coverage ending.
You must ask that we restore your coverage within 60 days of it ending. Also, the policy must not have ended for non-payment in the last 12 months.
We may restore a Cover Tennessee program. You must make your request in writing. Send the request within 30 days of when we tell you your coverage ended. The policy must not have ended for non-payment in the last 12 months.
These are only options if your policy ended because your monthly bills were not paid on time.
Please note: There may be a fee to restore the coverage. To restore COBRA, you may need to set up a bank draft. This will help make sure future payments are made on time.
