Follow-up after hospitalization for mental illness

Mental illness is the leading cause of disability in the United States. Regular follow-up ensures that patients transition successfully from the hospital to a community setting. Follow-up after hospitalization benefits both children and adults.

Patient population: People 6 years of age and older who were hospitalized for treatment of mental illness.

Quality Goal: Ensure patients receive timely follow-up care with a mental health practitioner as follows:

  • For BCBST and BlueCare patients, schedule an outpatient visit, an intensive outpatient encounter, or partial hospitalization with a mental health practitioner within 7 days of discharge.
  • For BlueCare patients, schedule an outpatient visit, an intensive outpatient encounter, or partial hospitalization with a mental health practitioner within 30 days of discharge.

What can you do for your patients hospitalized for mental illness?

  • When your patient is approaching discharge, call BCBST to assist with care coordination and transition of care services: 800-818-8581.
  • Impress upon your patients the importance of the appointment to monitor medications and other treatments and resources.
  • Contact and reschedule patients who miss their appointment.
