The Bureau of TennCare established a new TennCare Select program for persons with Intellectual Disabilities (ID) called SelectCommunity.  In April 2010, Arlington Class Members were transitioned into SelectCommunity.  The State-side and Self Determination DIDD waiver members are being phased into the Program.  The West Grand Region Members were given the opportunity to transition into SelectCommunity in August 2011.  The East Grand Region Members are being given the opportunity to transition in February 2012.  The Middle Grand Region Members are being given the opportunity to transition 3rd quarter 2011.  These dates are subject to change at TennCare’s discretion.

All SelectCommunity members are assigned a Nurse Care Manager (NCM) who serves as the member’s and provider’s primary point of contact for physical and behavioral health needs.  With the use of the Electronic Visit Verification system, (EVV), Nurse Care Managers are able to monitor the initiation and provision of home health and private duty services.
