BlueCross BlueShield of Tennessee contracted with LifeMasters® Supported SelfCare, Inc., to provide disease management services to eligible members.
Why participate?
Health is too important to leave to chance. Taking an active role in your own health positively impacts many aspects of your life. The new program helps put you in control of your disease.
What does the program do?
Chronic health conditions, when not kept under control, can lead to serious health complications. But they don’t have to. Disease management seeks to educate a patient on his or her condition and provide support to help keep that condition under control.
Who's eligible?
Members may be eligible if the program is part of their health plan and they are: between the ages of 18 and 64; or retirees under age 65 who have BlueCross BlueShield of Tennessee as their primary insurer.
