| Multiple Cancers, Including Cancer Syndromes (Hereditary) - Gene Panel - Upcoming Changes |
Ambulatory Care (AC) |
| To be effective December 31, 2026* |
Clinical Indications added/removed the following:
Current Role Remains Uncertain.[Multiple Cancers, Including Cancer Syndromes (Hereditary) - Gene Panels (less than or equal to 100 genes) may be indicated when ALL of the following are met:
- The member has a personal history, family history, tumor characteristic, or clinical phenotype suggestive of a hereditary cancer syndrome; and
- The requested genes are relevant to the member's documented clinical presentation, cancer history, family history, or suspected hereditary cancer syndrome; and
- Testing results are expected to directly impact clinical management, surveillance, treatment selection, risk-reducing interventions, or family risk assessment; and
- Documentation demonstrates that a syndrome-specific test or smaller targeted panel would not adequately evaluate the suspected hereditary cancer condition.] *
[Examples of Panels Reviewed Under This Guideline
The following examples are included but not limited to:
- Tempus xG+
- CancerNext Expanded
- CancerNext Expanded + RNAinsight
- Invitae Multi-Cancer Panel
- Empower Comprehensive
- MyRisk Comprehensive
- Custom hereditary cancer panels evaluating multiple unrelated hereditary cancer syndromes] *
Evidence Summary
Inconclusive or Non-Supportive Evidence
[Large hereditary cancer panels (greater than 100 genes) are considered investigational when one or more of the following apply:
- Testing is performed as broad history-agnostic screening without a documented hereditary cancer syndrome, phenotype, personal cancer history, or family history supporting evaluation
- The panel evaluates multiple unrelated hereditary cancer syndromes that are not supported by the member's clinical presentation
- Testing substantially exceeds what is required to evaluate the documented phenotype or suspected syndrome
- The panel includes numerous genes without established clinical utility, clinical validity, or management recommendations
- Documentation does not support a syndrome-specific indication for the requested testing.] *
