Clinical Indications added/removed the following:

Current Role Remains Uncertain.

[Myelodysplastic Syndromes (Somatic) - Gene Panels may be indicated when ALL of the following are present:

  • Molecular genetic testing utilizing DNA sequencing, RNA sequencing, RNA fusion analysis, or combined DNA/RNA next-generation sequencing panels (e.g., FoundationOne® Heme test, Tempus xT Heme plus xR, Archer FusionPlex Heme, Archer FusionPlex Pan-Heme, NeoGenomics Heme RNA panels, and comparable laboratory-developed RNA fusion assays) may be considered medically necessary when ALL of the following are met:
    • Individual has known or suspected Myelodysplastic Syndrome (MDS), Acute Myeloid Leukemia (AML), Acute Lymphoblastic Leukemia (ALL), Myeloproliferative Neoplasm (MPN), Chronic Myelomonocytic Leukemia (CMML), Mixed Phenotype Acute Leukemia, Multiple Myeloma, or other hematologic malignancy.
    • Clinical evaluation, laboratory testing, peripheral blood analysis, bone marrow examination, cytogenetics, and/or flow cytometry have been completed and additional molecular characterization is needed.
    • At a minimum ALL of the following genes must be included:
      • SF3B1
      • ASXL1
      • TET2
      • DNMT3A
      • RUNX1
      • SRSF2
      • U2AF1
      • TP53 ] *
